    Dear Hamilton Community,

     

    Below you will find some important information concerning the following:

    • Flu Shots for Families
    • First Day Packet Clarification
    • Free and Reduced Lunch Application Reminder


    Time for Flu shots/Protect your Community:

    All SPS students regardless of insurance can get a flu shot through any of the 30 school-based health centers. 

    Sign up with King County School Health at: https://kingcountyschoolhealth.com/about/#findSBHC

    Or All SPS staff and their families, and all SPS students and their families can get a flu shot through any one of 32 “Drive-through” Flu Shot Clinics starting Sept 21-Oct 2 at 16 different school locations. The “drive-through” flu clinics are sponsored by Seattle Visiting Nurse Association (SVNA) and SPS.

    Families can stay in their car but could also walk or bike up to the site.Children ages four and older can get the flu shot through the drive-through. 

    SVNA accepts most private insurance, Apple Health, Medicare, and credit cards. Families who do not have health insurance are also invited to sign up for a flu shot thanks the City of Seattle. Parents can enroll all family members for the same time slot.

    First Day Packet Clarification and Reminder to fill out and return the required “First Day Packet Forms”: 

    On our website under “Featured News” you’ll find a yellow tab that says, “Start of School Packet”.  Once you click that tab, this is what you will find shown below.  

    2020-21 Start of School Welcome Packet and General Information - Once you click this link you’ll find specific general information for Hamilton.

    The following required forms can be found on the SPS Start of School Forms page - When you click this link, it will take you to all of the required forms that need to be returned.

    • Student Health Information
    • Emergency Information and Student Release Form
    • Notification of Rights under the Family Educational Rights and Privacy Act (FERPA)
    • SPS Survey to Identify Disabled Students (504-2)
    • Title VI Student Eligibility Certification: Office of Indian Education

    Completed packets can be emailed to: hamilton1stdayforms@seattleschools.org. – This is our dedicated email address to return the forms.

    SEATTLE SCHOOLS FREE & REDUCED-PRICE MEALS:

    **A new application must be submitted every school year 

    APPLY:  Online via Paypams (Spanish & English) or Download the Free and Reduced Lunch Application including: Amharic, Chinese, English, Somali, Spanish, Vietnamese language versions. 

    If your family is eligible, please complete and return as soon as possible. If you qualify, your student(s) may also receive additional supports and benefits such as yearbook, ASB fees, event fees, public transportation assistance, field trips and College Bound Scholarship.
